About the Role

We are seeking an Emergency Department Case Manager - Care Navigator to join our team. This role will establish, coordinate, and maintain processes to increase patient throughput to the most appropriate level of care while facilitating interdisciplinary care across the continuum for the Emergency Department (ED). The successful candidate will collaborate with patients, families, multidisciplinary teams, physicians, community resources, and payers to ensure patient progress and care levels are determined appropriately.

Responsibilities
  • Acts as a resource to ED staff and physicians regarding appropriateness of admission, levels of care, quality of care concerns, and criteria utilized in care planning and resource utilization.
  • Gathers information from relevant sources to facilitate discharge from ED to the most suitable level of care, ensuring accuracy, safety, timeliness, and cost-effectiveness to prevent readmission and frequent visits.
  • Assists in triaging calls from other acute care facilities requesting patient transfers; determines transfer appropriateness by reviewing documentation and InterQual level of care criteria and discussing with admitting physicians, supervisors, and EMC administrators.
  • Assesses ED patients and identifies options other than acute hospital admission when feasible; screens and refers patients to acute rehabilitation, long-term acute care hospitals, nursing homes, clinics, or home-based treatments.
  • Organizes, integrates, and evaluates plan effectiveness; modifies plan as patient needs change to accomplish established goals.
Requirements
  • Bachelor's Degree in Nursing (BSN) or Master's Degree in nursing or healthcare-related field.
  • Registered Nurse (RN) licensure in the state of practice and Accredited Case Manager, Certified Case Manager, or Board Certification in Case Management.
  • Hospital Fire and Life Safety (HLFS) certification.
Estimated Salary Range

$80,000 - $110,000 per year, depending on experience and qualifications.
