Job Summary

We are seeking a highly skilled Pharmacy Clinical Specialist to join our team at Houston Methodist The Woodlands Hospital. As a key member of our pharmacy department, you will be responsible for providing advanced clinical pharmacy services to patients, ensuring optimal pharmacotherapy and promoting safe and efficacious medication use.

Responsibilities
  • Perform daily reactive and targeted proactive medication regimen reviews for patients within designated areas of responsibility or populations served.
  • Support the delivery of pharmacy-managed protocols, formulary processes, and medication reconciliation activities.
  • Preset educational programs for pharmacy staff and other hospital personnel on topics such as patient safety, medication management, and evidence-based practice.
  • Contribute to the design, analysis, and reporting of quality improvement program findings and participate in research projects to advance organizational quality of care and the pharmacy profession.
Requirements
  • Graduate of an education program approved by the credentialing body for required credentials.
  • Successful completion of an ASHP-accredited post-graduate year one (PGY1) residency training program or equivalent experience, or three years of clinical practice experience.
  • Meets HM standards of an ASHP-Qualified Residency Preceptor within one year of employment.
  • Certifications: Texas Pharmacist License, PPC- Pharmacist Preceptor Cert, ACLS, PALS, NRP, IV Certification, BLS.

  • Travel: May require travel within the Houston Metropolitan area and outside the Houston Metropolitan area.
Estimated Salary:

$125,000 per year.
