Job Overview

Tropical Health LLC is seeking a highly skilled and experienced Director of Nursing Operations to join our team. As a key member of our leadership team, you will be responsible for overseeing and directing all aspects of patient care for our home care patients.

Key Responsibilities:

  • Coordinate and supervise the overall provision of home care for hospice patients and their families.
  • Ensure that clinical records are maintained in compliance with state, federal, and agency standards.
  • Assist in the admission process and serve as a primary clinical point of contact to the Clinical Administrative Assistant.
  • Provide clinical expertise and assistance as needed to team members.
  • Facilitate documentation for certification, re-certification, and patient discharge/revocation within the EMR.
  • Support ongoing patient care needs by performing visits routinely.
  • Perform information visits and collect patient data for evaluation of new admissions.
  • Assist in providing administrative support in coordinating on-call nursing schedules.
  • Provide written evaluations of staff as necessary.
  • Monitor and adjust the caseloads of each clinical staff member and oversee the schedule of weekly visits in conjunction with the DON.
  • Facilitate healthcare and national recognition activities for patients at the direction of the DON.
  • Support volunteer activities as needed.
  • Coordinate and manage all DME-related activities and patient coordination.
  • Update and manage physician and facility profiles within the database in EMR.
  • Assist in orientation and training for AIDEs and LPNs, and support RN onboarding in conjunction with the DON.
  • Coordinate support from each IDG member as appropriate.
  • Manage the IDG notebook for compliance.
  • Ensure policy and procedures are followed.
  • Ensure effective communication for staff, patients, and families.
  • Assure that standards of ethical business and clinical practice are maintained.
  • Track and record falls monthly for DON review.
  • Ensure incident reports are printed for the notebook accordingly.
  • Monitor and ensure compliance with Federal and State regulatory agencies and other certifying bodies.
  • Report grievances and complaints made by patients or families to the appropriate persons.
  • Report all allegations of patient abuse and/or misappropriation of patient property.
  • Report all accidents and incidents.
  • Identify and respond appropriately to emergency situations.
  • Observe and ensure safety needs of patients.
  • Assist in the marketing of the hospice by highlighting the company as a community resource.
  • Act as the primary point of contact for ALFs, PCHs, LTACs, and make rounds routinely to maintain relationships.

Requirements:

  • A minimum of 3 years of nursing experience, hospice preferred.
  • Previous management experience.
  • Current and unrestricted nursing license; LPN or RN (RN preferred).
  • Ability to work well within an interdisciplinary team setting as well as independently.
  • Ability to communicate well with others both written and orally.
  • Ability to work a flexible schedule.

Working Conditions:

  • Be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids, and excrements.
  • Adverse environmental conditions and hazardous materials.

Transportation:

  • Must have a current valid driver's license, auto liability insurance, and reliable transportation.
