Job Overview

We are seeking a compassionate and skilled Registered Nurse to join our team as a Care Coordinator for Home Health and Hospice patients. As a member of our interdisciplinary care team, you will play a vital role in coordinating patient care and ensuring the highest quality of life for our patients.

Job Summary:

This is a challenging and rewarding opportunity for a Registered Nurse to work in a dynamic home health and hospice environment. You will be responsible for assessing patient needs, developing personalized care plans, and collaborating with healthcare teams to ensure seamless care coordination.

Main Responsibilities:
  • Patient Assessment and Planning: Conduct thorough assessments of patients' physical, emotional, and social needs, and develop comprehensive care plans that address their unique requirements.
  • Care Coordination: Collaborate with healthcare teams, including physicians, therapists, and other caregivers, to ensure timely and effective delivery of care services.
  • Communication and Education: Provide clear and concise information to patients, families, and caregivers about treatment options, medication management, and disease progression.
  • Documentation and Reporting: Accurately document patient interactions, progress notes, and care plans in electronic medical records.
Requirements:
  • Licensure and Certification: Current RN licensure in the state where services are provided; BLS certification from American Heart Association (AHA)
  • Education: Graduate of an accredited school of nursing or equivalent education
  • Experience: No experience required; willingness to learn and adapt to new situations
Work Environment:

This position requires frequent travel to patients' homes, hospitals, or other healthcare facilities. You must be comfortable working in a fast-paced environment and able to navigate various settings with ease.
