At Urrly, we're seeking a dedicated Nurse Practitioner to lead our groundbreaking care model, delivering exceptional primary care to a dedicated panel of patients. As a champion of the 'treat in place' model, you'll ensure advanced care planning is seamless and tailored to each individual.
Your Mission:
- Deliver personalized care to patients, focusing on their unique health goals.
- Champion the 'treat in place' model, ensuring advanced care planning is seamless and tailored to each individual.
- Conduct thorough assessments, create personalized care plans, and coordinate with care teams to ensure the best possible outcomes.
What You'll Bring:
- Unrestricted state licensure as an Advanced Practice Nurse and national board certification.
- At least 6 months of experience as an Advanced Practice Provider, preferably with geriatric, hospice, or palliative care populations.
- Strong clinical skills and experience in treating chronically ill patients, with a preference for nursing home environments.
