Ohel Children's Home and Family Services is a dynamic organization seeking a skilled Registered Nurse to join its field-based care transition team. This role offers an exciting opportunity for a compassionate healthcare professional to make a meaningful impact in the lives of individuals and families.
Key Responsibilities- Conduct Comprehensive Assessments: Conduct thorough health assessments, documenting patient information accurately and maintaining up-to-date records.
- Collaborate with Healthcare Teams: Work closely with interdisciplinary teams to develop comprehensive care plans, ensuring seamless transitions and optimal patient outcomes.
- Educate Patients and Families: Educate patients on effective health maintenance strategies, disease prevention techniques, and healthy lifestyle choices.
- Monitor Patient Conditions: Continuously monitor patients' conditions, reporting any changes or concerns to ensure timely interventions.
- Provide Counseling Sessions: Offer counseling sessions under the supervision of senior staff members, providing emotional support and guidance to patients and their families.
- Licensure and Experience: Possess a current New York State RN license and at least one year of clinical experience, preferably in behavioral health settings.
- Knowledge and Skills: Demonstrate knowledge of health promotion and disease prevention principles, recognizing abnormal signs and symptoms. Familiarity with medications and potential adverse reactions is essential.
This full-time position is located in Queens and offers a hybrid schedule, with inpatient, home, and community visits required. The ideal candidate will be a self-starter who can work independently as well as within a team, possessing excellent follow-up skills and a strong commitment to delivering exceptional patient care.
