About the Job:

  • This position requires a licensed practical nurse to provide skilled nursing services to patients in our hospice program.
  • The ideal candidate will have experience working in a healthcare setting and be knowledgeable about computer office/clinical software.
  • Associate's in Nursing or Diploma preferred, with current IV Certification required.
  • A clear and active Licensed Practical Nurse license to practice in the state of Florida is essential.
  • Must have 1-2 years of healthcare or clinical nursing experience and be able to work a flexible Full-Time schedule that includes weekends, evenings, and holidays.
  • We offer a competitive salary of $55,000 - $65,000 per year, depending on experience.

Job Description:

">Responsibilities:">
  1. Provide skilled nursing care to patients in accordance with agency policies and physician orders.
  2. Promote and maintain patient quality of life under the guidance of the registered nurse.
  3. Assist patients in learning appropriate self-care techniques.
  4. Help achieve and maintain continuity of patient care by assisting in planning and exchanging information with other health professionals.
  5. Prepare clinical visit notes and follow client care plans as developed by the registered nurse and according to agency policy.
  6. Assist the physician and registered nurse in performing specialized procedures for which competency has been established.
  7. Make visits as necessary according to assessed needs, within established time frames.
  8. During patient visits, provide necessary education to patient/caregiver/family to enable them to provide appropriate care for the patient.
  9. Participate in the nursing process of assessing, planning, implementing, and evaluating patient care under the direction of the supervising RN, Clinical Care Manager.
  10. Reports results of visits to Clinical Care Manager or Triage Manager.

Requirements:

  • Associate's in Nursing or Diploma.
  • Clear and Active Licensed Practical Nurse license to practice in the state of Florida.
  • Current IV Certification required.
  • 1-2 years of healthcare or clinical nursing experience preferred.
  • Must have knowledge of computer office/clinical software.
  • Must be able to read, write, and understand the English language.
