Responsibilities of a Home Health RN Field Nurse
  1. Complete an initial assessment of patient and family to determine home care needs.
  2. Provide a complete physical assessment and history of current and previous illness(es).
  3. Supervise and direct the Licensed Practical Nurse (LPN) and/or the Home Health Aide in implementing the patient care plan.
  4. Develop patient education plans, as needed, and direct the LPN in implementing said plans.
  5. Create and modify the Home Health Aide care plan and supervise them via patient feedback and onsite interactions.
  6. Evaluate and revise plan of care as patient status and needs change.
  7. Use health assessment data to determine nursing diagnosis.
  8. Develop a plan of care, which establishes goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative action.
  9. Administer medications and treatments as prescribed by the physician.
  10. Identify discharge planning needs as part of the care plan development and implementing prior to discharge of the patient.
  11. Act as the Case Manager when assigned by the Clinical Supervisor and assume responsibility to coordinate patient care for assigned caseload.
  12. Prepare clinical notes as needed and communicate with the physician regarding patient's needs.

Education, Licensure & Certification Requirements:

  • Graduate of an accredited school of nursing.
  • Active RN licensure.
  • One (1) to two (2) years of recent acute care experience in an institutional setting.
  • Bachelor's degree with one (1) year of home health care experience preferred.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
