Job Summary

We are seeking a highly skilled Hemodialysis Nurse Specialist to provide patient education and support in our home hemodialysis program.

Key Responsibilities

  • Train patients and their caregivers in self-care dialysis procedures, ensuring they understand the treatment process and can manage it independently.
  • Conduct regular home visits to assess the patient's environment and adjust the treatment plan as needed.
  • Monitor patient performance and report any concerns or issues to the physician and care team.
  • Document all nursing services, including training sessions, patient interactions, and phone conversations, in the Electronic Medical Record.
  • Participate in infection control monitoring, implementation, and recording, as well as emergency preparedness procedures.
  • Ensure clinic and patient supply inventory is accurate and ordered in a timely manner to avoid fees and penalties.
  • Continuously seek opportunities to improve patient outcomes through data tracking, critical thinking, and quality improvement activities.
  • Administer medications as ordered by the physician and perform on-call nursing services as needed.

Requirements

  • RN licensure and experience in hemodialysis or home health preferred.
  • Strong communication and interpersonal skills.
  • Able to work independently and as part of a team.
