Job OverviewWe are seeking a skilled Nurse Specialist in Chronic Care to join our team. This role will involve working with patients who require ongoing care and treatment for chronic conditions. As a key member of our healthcare staff, you will be responsible for delivering high-quality patient care and collaborating with other medical professionals to ensure the best possible outcomes.
About the PositionThis is a full-time opportunity with a competitive salary range of $85,000 - $115,000 per year, depending on experience. The position requires 12 weeks of commitment, working 40 hours per week with flexible shift arrangements. You will have the opportunity to work in a dynamic and supportive environment, collaborating with a multidisciplinary team to deliver exceptional patient care.
The ideal candidate will possess a strong background in nursing, with at least 1 year of experience in nephrology nursing. A current RN license and CPR BLS certification are required. Key Responsibilities:
- Provide direct patient care, including assessment, planning, intervention, and evaluation
- Collaborate with other medical professionals to develop and implement individualized care plans
- Conduct regular monitoring and evaluation of patient progress, making adjustments to care plans as necessary
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
- Participate in quality improvement initiatives and contribute to the development of policies and procedures
Requirements:
- RN licensure and current CPR BLS certification
- At least 1 year of experience in nephrology nursing
- Strong understanding of chronic care principles and practices
- Excellent communication and interpersonal skills
- Ability to work collaboratively as part of a multidisciplinary team
