The Role

We are seeking a skilled Licensed Practical Nurse to join our team on a NOC shift. As a key member of our multi-disciplinary team, you will provide direct nursing care, including medication administration, phlebotomy, and documentation. Your primary responsibility will be to support the needs of our clients, ensuring their safety and well-being.

Responsibilities

  • Conduct daily physical and mental status assessments and document them accurately.
  • Administer medication, monitor side effects, and document client responses.
  • Manage client health questionnaires, vital signs, and weight checks, ensuring accuracy and reporting any unusual readings.
  • Participate in client observations to promote abstinence and safety.
  • Coordinate with pharmacy for medication deliveries and ensure HIPAA compliance.
  • Monitor client nutritional and dietary needs, reporting non-compliance with the care plan.
  • Organize and coordinate consults with physicians and medical personnel for tests and diagnostic procedures.
  • Complete Alsana Nursing Assessments, Care-plans, S.O.A.P Notes, Admission Notes, Discharge Notes, and NOC Shift Notes, ensuring accurate documentation.
  • Administer first aid according to facility policy and document follow-up actions.
  • Obtain insurance updates for precertification and maintain good communication with clients, families, and staff.
  • Perform all other duties as assigned.

Qualifications

  • Valid license as a Practical Nurse in the state of employment.
  • 1+ years of hospital-based clinical experience preferred.
  • Current CPR Certification through the American Heart Association.
  • 1+ years of clinical experience in a mental health or residential facility preferred.

Hourly Rates:

  • LPN: $25.00.
  • RN: $33.00.

This position is eligible for shift differential pay.

Schedule: NOC Friday, Saturday, and Sunday, 7:00 PM to 7:00 AM CT.
