Job Summary

We are seeking a highly skilled Registered Physical Therapist to join our team at Granite Peak Home Health. As a Physical Therapist, you will play a vital role in providing high-quality patient care and improving the lives of our clients.

Key Responsibilities
  • Administer physical therapy to patients in their place of residence, as ordered by physicians and in accordance with established plans of care.
  • Conduct initial assessments, develop and implement maintenance programs, and make modifications and reassessments as needed.
  • Direct and supervise personnel, as required, including Physical Therapy Assistants and Home Health Aides.
  • Consult with physicians regarding changes in treatment and write reports to physicians regarding patient progress.
  • Participate in in-service programs and present in-service programs, as assigned.
  • Participate in Quality Assurance and Performance Improvement activities, as assigned.
  • Attend all patient care conferences, as scheduled.
  • Prepare clinical and progress notes.
  • Assist physicians with evaluating patient function and help develop and revise plans of care, as necessary.
  • Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition, in accordance with Agency-defined time frames.
  • Appropriately utilize ICD-10 codes.
Requirements
  • Hold a current, unencumbered license in the state(s) in which practicing.
  • Current driver's license.
  • Two (2) years experience preferred in Home Health.
  • Knowledge of home health regulations, procedures, laws, regulations, and guidelines pertaining to home health care.
  • Caring attitude, observance, maintaining good working relationships, judgment, and emotional stability.
