Job Summary: We are seeking a dedicated and compassionate Nurse Assistant to join our team at vTech Solution, Inc. in Seattle, WA. As a Clinical Care Coordinator, you will provide high-quality patient care and support in our Transitional Care Unit (TCU).

**Responsibilities:**
• Assist patients with daily living activities, such as bathing, dressing, and grooming
• Monitor vital signs and report any changes to nursing staff
• Maintain accurate records of patient information and medication administration
• Collaborate with interdisciplinary teams to develop and implement individualized care plans
• Provide emotional support and education to patients and their families

Requirements:
• 2 years of experience in skilled long-term care or transitional care settings
• Current BCLS certification
• Ability to work every other weekend and have reliable transportation
• Strong communication and teamwork skills
• Ability to adapt to changing situations and prioritize tasks effectively

Compensation: $45,000 - $55,000 per year, depending on experience.
