Caregiver and Patient Advocate Role

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Abbotts Creek Center. In this role, you will be responsible for delivering high-quality patient care under the supervision of a Registered Nurse.
Job OverviewThis position is ideal for individuals who are passionate about providing compassionate and personalized care to patients in a long-term care setting. As an LPN, you will work collaboratively with our interdisciplinary team to ensure seamless care delivery.
Responsibilities- Evaluate patients/residents to collect data, observe condition, and report changes in condition.
- Contribute to nursing assessments and care planning.
- Administer medications and perform treatments per physician orders.
- Supervise and monitor patient care provided by unlicensed staff.
- Must be a graduate of an approved school for practical or vocational nursing.
- Must be currently licensed by the State Board of Nursing or pending licensed practical nurse licensure.
- Must maintain current BLS/CPR certification if employed at a skilled nursing center or as required by the state of practice if an assisted living facility.
