We are seeking a dedicated and compassionate Healthcare Professional to join our team at Kaleida Health.

Job Summary

This position provides comprehensive care to residents in a long-term care setting, promoting their independence and well-being through individualized care plans. The successful candidate will work collaboratively with licensed personnel to deliver high-quality patient care and maintain a safe and supportive environment.

Key Responsibilities:

  • Provide direct resident care, including assistance with activities of daily living (ADLs) and ensuring residents' dignity and comfort
  • Develop and implement individualized care plans, collaborating with interdisciplinary teams to meet residents' unique needs
  • Maintain accurate and thorough records of resident care, including progress notes and medication administration
  • Respond promptly to resident calls for assistance, providing timely support and addressing concerns

Requirements:

  • High School Graduate or GED preferred
  • Successful completion of a Certified Nurse Aide program and current NYS certification
  • Experience working in long-term care or a related field is an asset
