About the Role

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team in providing high-quality home health care services to clients in Aiken, SC.

As an LPN with Professional Case Management, you will have the opportunity to work independently, creating your own schedule that suits your needs. This flexibility allows you to maintain a healthy work-life balance while making a positive impact on the lives of our clients.

Key Responsibilities
  • Assess client needs and implement personalized care plans to ensure optimal health outcomes
  • Provide nursing care and support to clients in their homes, promoting independence and dignity
  • Maintain accurate records and communicate effectively with clients, families, and healthcare teams
  • Evaluate client response to interventions and adjust care plans as needed
Requirements
  • Graduate of a state-approved school of professional nursing
  • Current, unrestricted license as an LPN in the state(s) of practice
  • New grads welcome
  • Current CPR certification and TB test
