Job Title: Health Information Manager
Job Summary:
Aviva at Fitzsimons is seeking a highly skilled Health Information Manager to join our team. The successful candidate will be responsible for managing all functions within the Medical Records department, ensuring the accurate and secure storage of resident medical records.
Key Responsibilities:
- Maintain accurate records of daily and monthly census
- Complete required audits, including Admission, Discharge, Re-admission, Quarterly audit, and Physician visit
- Accurately maintain organized paper and electronic charts for current and discharged residents
- Scan documents into PointClickCare to facilitate electronic records
- Maintain current forms and make them available to the Interdisciplinary Team (IDT)
- Obtain hospital discharge summaries for residents admitted and re-admitted from the hospital
- Maintain a diagnosis list with proper codes and ensure updates as changes occur
- Ensure proper diagnosis coding according to ICD-10 CM Coding guidelines
- Update daily mailing and filing, and track medical professionals' signatures on orders and other required documents
- Maintain current credentials for outside medical professionals providing care to residents
- Maintain signature logs for medical and nursing staff
- Ensure medical information is released according to HIPAA standards and keep the ROI log current, informing the Nursing Home Administrator (NHA) and Director of Nursing (DON) of any ROI requests
- Provide training and education to the IDT on HIPAA privacy and security regulations
- Attend monthly QAPI meetings and prepare as needed
- Attend weekly Medicare meetings and prepare as required
- Attend triple check and prepare as required
- Complete physician certification and recertification for all Medicare and managed care Part A services
- Ensure timely and compliant issuance of Medicare notices, maintaining a working knowledge of required current notices
- Perform data entry of Accident and Incident reports as required
- May supervise department employees, including hiring, firing, evaluating, coaching, scheduling, etc.
- Collaborate with physicians, therapists, consultants, hospital personnel, and nursing staff, as well as other supervisors and staff within the facility, regarding resident care documentation and corporate HIM consultant
- Scan all documents into PointClickCare
- Perform all other duties as assigned
Requirements:
- High school diploma required; two years of HIM experience and two years of supervisory experience preferred
- Knowledge of medical terminology, computer skills, and general office equipment
- Familiarity with state and federal regulations
Work Environment:
- Office environment with occasional exposure to healthcare settings
- Requires frequent communication with personnel, facility staff, and other department supervisors
- Subject to hostile and emotionally upset individuals
- Works beyond normal duty hours, on weekends, and holidays as necessary
- Subject to call-back during emergency conditions
Physical and Sensory Requirements:
- Specific vision abilities required, including close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus
- Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile individuals
- Must be able to make independent decisions when circumstances warrant such action
- Must be able to concentrate and use reasoning skills and good judgment
- May be necessary to assist in the evacuation of residents during emergency situations
- Must be able to sit, stand, bend, lift, push, pull, stoop, walk, use hands to finger, handle or feel objects, tools, or controls; reach, climb stairs, talk, and hear
- Must be able to occasionally lift 30 lbs. floor to waist
