We are seeking a highly skilled and dedicated Registered Nurse to join our team at Constructive Partnerships Unlimited. As a Lead Healthcare Professional, you will play a crucial role in providing exceptional care and support to individuals with disabilities.

About the Role

This is an exciting opportunity for a Registered Nurse with a passion for disability support to take on a leadership role and make a meaningful difference in the lives of our clients. You will be responsible for developing and implementing individualized healthcare plans, providing direct care and supervision, and collaborating with multidisciplinary teams to ensure seamless care delivery.

Key Responsibilities
  • Assess the needs of individuals with disabilities and develop personalized healthcare plans to meet their unique requirements.
  • Provide direct care and supervision to Licensed Practical Nurses on shift, ensuring that high-quality care is delivered consistently.
  • Oversee the implementation of healthcare plans, including training staff and individuals with disabilities on health-related procedures and protocols.
  • Evaluate the effectiveness of healthcare plans and make adjustments as needed to ensure optimal outcomes.
  • Collaborate with other healthcare professionals, including physicians and nurses, to coordinate care and share best practices.
  • Maintain accurate records and documentation of client care, including progress notes and medication management.
Requirements
  • NYS Licensed Registered Nurse (BSN minimum) with a strong background in disability support.
  • Minimum 2 years of experience working with individuals with disabilities, preferably in a leadership or supervisory role.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • CPR Certification and commitment to ongoing professional development.
What We Offer

We offer a competitive salary of $65,000 per year, based on your qualifications and experience. In addition, you will have access to ongoing training and professional development opportunities, as well as a supportive and collaborative work environment.

The estimated salary range for this position is $60,000 - $70,000 per year, depending on your level of experience and qualifications.
