Pediatric Home Care Nurse Specialist

We are seeking a highly skilled Pediatric Home Care Nurse to join our team at Hospice of the Western Reserve.

Your Role
As a Pediatric Home Care Nurse, you will provide compassionate and high-quality care to pediatric patients and their families in the comfort of their own homes.
Responsibilities:
- Assess the physical, psychosocial, and spiritual needs of patients and families.
- Provide intermittent skilled nursing services under the direction of the patient's physician.
- Monitor and manage pain and symptoms, providing education and support to patients and families.
- Serve as a liaison between patients, physicians, and other healthcare professionals.
- Provide emotional support and counseling to patients and families.
- Refer patients and families to additional resources and services as needed.
- Develop and implement personalized care plans to meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination.
- Maintain accurate and timely documentation of patient care activities.
- Participate in quality improvement initiatives to enhance patient outcomes.
Requirements:
- Licensed RN in the State of Ohio.
- BS or equivalent experience.
- 3-5 years of experience in Pediatrics or Hospice, with a desire to transfer into Pediatrics.
- Experience in pain and symptom management, oncology, public health, and mental health preferred.
- Knowledge of pediatric assessment skills, pharmacology, and normal growth and development.
- Ability to work a minimum of 40 hours per week, with flexibility to accommodate department needs.
- Valid driver's license, reliable vehicle, and insurance coverage required.
- Ability to travel within designated service areas.

Estimated Salary Range: $80,000 - $110,000 per year, based on experience and location.
Working Conditions:
- Primarily office-based, with occasional travel to patient homes and healthcare facilities.
- Ability to lift 25 pounds and stand for extended periods.
- Must pass annual flu shot requirement or provide exemption documentation.
