Job Summary:

We are seeking a skilled Nurse Practitioner to join our team at AlignMed Partners. As a Nurse Practitioner - Sub-Acute Care Expert, you will provide primary and urgent care services to nursing center residents in collaboration with other medical providers.

Responsibilities:

  • Provide high-quality patient care to residents in a sub-acute care setting
  • Collaborate with the Center Leadership Team to achieve AlignMed Partners' mission of improving the lives we touch through the delivery of high-quality healthcare and everyday compassion
  • Work collaboratively with Medical Directors and Attending Physicians to provide patient-specific medical care

Requirements:

  • Graduate from an accredited School of Nursing program
  • Licensed as a registered nurse
  • Certified as a Nurse Practitioner by being a nursing graduate from an accredited Nurse Practitioner Program with a Master's Degree in Nursing and a concentration and specialty track of Older Adult and/or Family Nurse Practitioner (FNP)
  • A minimum of two years of experience working with adult or geriatric populations
