Job Summary

We are seeking a highly skilled Speech Language Pathologist to join our team at HealthPRO Heritage. As an SLP, you will be responsible for providing and directing speech therapy services to patients in our assisted and independent living communities.

Key Responsibilities:

  • Evaluate residents to determine and diagnose communicative deficits and equipment needs.
  • Develop and implement individualized treatment programs based on initial evaluations and containing achievable functional goals.
  • Conduct and document thorough assessments of residents to determine and diagnose communicative deficits and equipment needs.
  • Enter progress notes in resident records following each treatment session.
  • Revise treatment programs as needed based on resident performance.
  • Document the course of treatment in clear, concise discharge summaries.
  • Orient residents to program services and treatment procedures appropriate to their needs.
  • Work with nursing staff, residents, and families to maximize residents' use of functional abilities.
  • Assist in the referral process when residents require additional services following discharge.
  • Attend and participate in rehabilitation team conferences regarding resident progress, problems, or needs.

Requirements:

  • Masters Degree in Speech and Language Therapy.
  • Hold a current Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) credential offered by the American Speech-Language-Hearing Association.
  • Assumes responsibility for ongoing continuing education and professional development.
  • Must maintain valid state Speech Language Pathology licensure.
