Per Diem Physical Therapist Role in Home Healthcare

At Mass General Brigham, we are seeking a skilled Per Diem Physical Therapist to join our dynamic Home Hospital (HH) team. As a member of this innovative program, you will provide acute-level care to patients in the comfort of their own homes.

About the Role

This Per Diem Physical Therapist position will be part of a clinical team that provides comprehensive physical therapy services to patients with physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. You will work collaboratively with patients, families, physicians, and other healthcare professionals to identify patient needs and coordinate services to meet those needs.

Key Responsibilities
  • Develop personalized plans of care that are patient and family focused, incorporating standardized interventions for goal-oriented outcomes.
  • Conduct virtual and in-person patient evaluations, applying evidence-based practice and established HH criteria.
  • Maintain accurate and timely documentation, promoting real-time updates visible to the full clinical care team.
  • Implement standard treatment procedures, teaching patients and caregivers as necessary.
  • Complete age-appropriate evaluations, reflecting patient history, relevant systems review, and appropriate tests and measures.
Requirements and Qualifications
  • Minimum of two years' experience as a licensed Physical Therapist, with homecare and/or acute care experience preferred.
  • Registered with the Board of Registration in Massachusetts.
  • Strong customer service skills and ability to work with diverse populations.
  • Excellent verbal, written, and electronic communication skills.
  • Computer proficiency and ability to learn new software; Epic and Rover experience preferred.
  • Ability to prioritize work and adapt to changing situations.
Compensation and Benefits

The estimated salary range for this role is between $85,000 to $120,000 per year, depending on experience. Benefits include medical benefits, dental benefits, vision benefits, life insurance, and employee assistance programs.
