Key Responsibilities

Coordinate PPS data collection with the interdisciplinary team to ensure accurate and timely completion of IRF-PAI forms

Ensure completion of supportive documentation for FIMs in medical records

Complete admission and discharge IRF-PAI forms within CMS-designated time frames

Collect and summarize relevant information from medical records for IRF-PAI forms

Assist in team education regarding FIM/PPS and documentation best practices

Requirements

High school diploma or equivalent required; bachelor's degree in a healthcare-related field or licensed healthcare provider preferred

Three to five years of experience in a healthcare setting, with strong communication and organizational skills

Ability to read, write, and speak English fluently

Strong reasoning ability and independent judgment

Ability to present a professional image and work effectively in a fast-paced environment
