Home Health LPN

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in La Crosse and the surrounding areas. As a Home Health LPN, you will provide high-quality clinical services to patients in their homes, working closely with the healthcare team to ensure seamless care.
Responsibilities- Provide direct nursing care as prescribed by physicians to acutely ill and chronically ill patients
- Record clinical and progress notes, and coordinate other patient care activities/services to deliver exceptional patient care
- Work under the supervision of a Registered Nurse or designated supervisor, adhering to state laws governing nursing practice, agency policies, and procedures
- Current LPN licensure in the state of practice
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
- Current CPR certification required
- Previous home care experience preferred
- A competitive salary: $65,000 - $80,000 per year
