About the Role

We are seeking a highly skilled Senior Nursing Care Professional to join our team at Nuvance Health.

Job Summary

The Registered Nurse provides direct patient care that is guided by the American Nurses Association (ANA) scope of practice. They collaborate with patients, family members, peers, physicians, and other healthcare professionals in delivering high-quality patient care services.

Main Responsibilities

  • Conduct thorough patient assessments and reassessments as clinically indicated:

a. Document detailed information incorporating the plan of care, interventions, and outcomes reflecting the patient's physical, mental, and emotional status.

b. Ensure accurate documentation based on the level of care the patient is receiving, including transfer/discharge documentation as necessary.

  • Adhere to the five Rights of Medication Administration. Clearly communicate medication information to patients/family members. Assess patients prior to administering medications, noting allergies, parameters, potential side effects, effectiveness, and intervening as needed. Reassess and document response according to hospital policy.
  • Coordinate and supervise the delivery of nursing care, including delegating tasks:
  • Maintain accountability for delegated care.
  • Foster a culture of safety:
  • Assess patients for potential safety risks.
  • Comply with hospital policies and procedures regarding patient safety. Initiate interventions related to identified risk factors.
  • Conduct bedside shift reports and rounds with peers on assigned patients at change of shift.
  • Demonstrate quality by documenting the application of the nursing process in a responsible, accountable, and ethical manner.
  • Ensure smooth patient handovers when leaving for breaks.
  • Assess learning needs of patients/family members. Develop and provide appropriate instruction using applicable teaching methods. Evaluate and document patient/family member response to teaching.
  • Care for specific patient populations, regardless of location, as necessary.
  • Prioritize Patient-Centered Care:
  • Show respect for patients' cultural diversity and differences. Maintain dignity through the caring relationship.
  • Maintain open lines of communication, actively listening.
  • Meet accreditation, regulatory, and compliance responsibilities related to the position.
  • Foster a culture of growth and learning:
  • Seek experiences and formal/independent learning activities to maintain and develop clinical/professional skills/knowledge.
  • Teach others through mentorship.
  • Perform other duties within the state nursing scope of practice as assigned.

  • Demonstrate regular, reliable, and predictable attendance.
  • Complete other tasks as required.

Education and Experience Requirements

  • Bachelor's Degree in Nursing or Associate's Level Degree from an accredited school of nursing (required).
  • Licensure/Registration/Certification: Current, unobstructed New York State Registered Nurse (NY Hospitals) license, Current and unobstructed Connecticut Registered Nurse License (CT Hospitals), or awaiting licensure at the discretion of the Chief Nursing Officer (CT only).
  • American Nurses Association endorsed Certifications or Academy of Medical-Surgical Nurses Certification preferred (*Based on Certification specialty hours of work requirement).
