Pediatric Clinical Pharmacist Job Description

At Children's Health, we are seeking a highly skilled and dedicated Pediatric Clinical Pharmacist to join our team. As a key member of our multidisciplinary healthcare team, you will play a vital role in promoting safe, rational, effective, and efficient medication use for our pediatric patients.

Responsibilities:
  • Review and verify medication orders to ensure appropriateness and accuracy.
  • Assess the effectiveness, safety, and affordability of patient medications, working collaboratively with the healthcare team to develop and optimize patient medication therapy.
  • Provide expert drug information and pharmacotherapy consultations to physicians, nurses, and patients/families.
  • Monitor inpatient and outpatient profiles for drug interactions, food-drug interactions, and duplication of therapy.
  • Participate in transitions of care and assist with issues involving patient access to medications.
  • Contribute to quality improvement activities, including floor inspections, medication use evaluations, and documentation of interventions.
  • Support the development, implementation, and ongoing review of Epic order entry and set build.
  • Attend hospital committee meetings as needed.
  • Participate in continuing education programs, in-services, and workshops to maintain current knowledge and enhance professional growth.
Requirements:
  • At least 1 year of experience in hospital pharmacy.
  • Completion of a pharmacy practice residency with pediatric experience (preferred).
  • Pediatric experience in clinical specialty areas (preferred).
Education:
  • Bachelor's degree in pharmacy (BS Pharm) or Doctorate of Pharmacy (PharmD) from an accredited school of pharmacy.
  • Completion of a pediatric clinical pharmacy residency (preferred).
Licenses and Certifications:
  • Current Texas Pharmacist license.
  • Pharmacist Preceptor license (required within 90 days).
  • Basic Life Support for Healthcare Providers (required within 90 days).
  • 20-hour ACPE-accredited IV training course (required within 90 days).
  • Board of Pharmacy Specialties Certification (preferred).
