Job Summary: We are seeking a highly skilled LPN Patient Care Manager Assistant to join our team at Assured Home Health, a part of LHC Group. As a key member of our home health team, you will be responsible for assisting the Patient Care Manager in coordinating a team of staff to provide high-quality care to patients in their homes.

Responsibilities:

  • Assist the Patient Care Manager in coordinating a team of staff to ensure continuity of care to patients assigned to the team's area.
  • Ensure that the team is committed to providing quality care in an efficient manner in accordance with physician's orders and State/Medicare guidelines.
  • Prepare patient lists and necessary documents for weekly case conferences.
  • Provide direct patient care, as necessary, in accordance with scope of practice, plan of care, and physician orders.
  • Assist the Patient Care Manager in clinician documentation review/audits to ensure complete, accurate, and timely submission of all documentation by field staff.

Requirements:

  • Current LPN licensure in the state of practice.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
