Job Overview:



We are seeking a skilled Nurse Care Manager to join our team at CHRISTUS Santa Rosa Hospital - New Braunfels. As a key member of our nursing staff, you will provide high-quality patient care and support the hospital's mission to deliver exceptional healthcare services.



About the Role:




  • Assess patients' physical, psychosocial, social, cultural, spiritual, and development needs
  • Develop and implement individualized plans of care using evidence-based practice
  • Collaborate with healthcare teams to ensure seamless patient care
  • Maintain accurate and detailed patient records
  • Participate in quality improvement initiatives to enhance patient outcomes


Requirements:




  • Bachelor's Degree in Nursing (BSN) or equivalent experience
  • 1 year of experience in a related nursing specialty (preferably)
  • RN license in the state of employment or compact
  • BLS certification required


Compensation and Benefits:



We offer a competitive salary of $95,000 per year, commensurate with experience. Additionally, we provide a comprehensive benefits package, including medical, dental, and vision insurance, retirement savings plan, paid time off, and professional development opportunities.
