Job Summary

We are looking for a skilled Nurse to coordinate the Case Management Program, ensuring that clients receive timely and cost-effective home and community-based services. The successful candidate will work closely with service providers, clients, and a Licensed Social Worker case manager to determine eligibility and provide support.

Responsibilities

  • Coordinate the Case Management Program, planning and arranging for appropriate services
  • Maintain contact with service providers, clients, and collaborate with the case management team
  • Determine client eligibility and provide necessary support

Requirements

To be successful in this role, you will need:

  • A valid nursing license
  • Excellent communication and interpersonal skills
  • The ability to work independently and as part of a team

Benefits

We offer a competitive salary range of $50,000 to $55,000 per year, along with excellent fringe benefits. This position does not involve nights, weekends, or on-call shifts, providing a great work-life balance.
