Job Summary

We are seeking a skilled Clinical Care Coordinator to join our team. This role will be responsible for coordinating and directing patient care activities, ensuring seamless transitions between departments and providing exceptional patient experiences.

Key Responsibilities

  • Plan, coordinate, and direct patient care activities to ensure efficient and effective care delivery.
  • Collaborate with interdisciplinary teams to develop and implement patient care plans.
  • Communicate effectively with patients, families, and healthcare teams to ensure clear understanding of patient needs and preferences.
  • Identify and address potential patient safety risks and concerns.
  • Develop and implement process improvements to enhance patient care and satisfaction.
  • Provide education and training to staff on patient care best practices and policies.
  • Participate in quality improvement initiatives to ensure high-quality patient care.

Requirements

  • Graduate of an accredited School of Professional Nursing.
  • Current State of Texas license or temporary/compact license to practice professional nursing.
  • 10 years of nursing experience in a related field.
