We are seeking a skilled Registered Nurse to fill the position of Case Manager at Mass General Brigham in Hadley, Massachusetts. As a key member of our team, you will be responsible for providing high-quality care and case management to patients and their families.

Key Responsibilities
  • Provide skilled care and case management to patients and their families in accordance with the State Nurse Practice Act and accepted professional standards of practice.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Collaborate with interdisciplinary teams to ensure seamless coordination of care and services.
Requirements

To be successful in this role, you must possess:

  • A current license to practice as a Registered Nurse in the Commonwealth of Massachusetts.
  • A minimum of one year Medical/Surgical nursing experience.
  • Excellent verbal and written communication skills and strong interpersonal skills.
  • Demonstrated organizational and time management skills.
  • A current driver's license and reliable transportation.
Salary and Benefits

The salary range for this position is $85,000 - $110,000 per year, depending on experience. Additionally, we offer a comprehensive benefits package including medical, dental, vision, life insurance, and employee assistance programs.
