At Touchstone Communities, we're seeking an experienced and compassionate Licensed Vocational Nurse (LVN) to join our team as a Charge Nurse. This role is perfect for individuals who are passionate about delivering exceptional patient care and leading a dedicated team.
Job SummaryAs a Charge Nurse at Heritage Nursing and Rehab, you will be responsible for providing comprehensive nursing care to our residents while ensuring the smooth operation of our 2pm to 10pm shift. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident.
Key Responsibilities- Provide direct patient care to assigned residents, including administering medications, performing vital signs, and assisting with daily living activities;
- Lead and manage a team of nursing staff, providing guidance and support to ensure excellent patient care;
- Develop and implement individualized care plans that meet the unique needs of each resident;
- Collaborate with other healthcare professionals to coordinate care and services;
- Maintain accurate and up-to-date medical records, adhering to regulatory requirements;
- Participate in quality improvement initiatives and contribute to a culture of safety and excellence.
To be successful in this role, you must possess:
- A current and valid Texas LVN nursing license or a current and valid LVN/LPN license from a Compact Party State;
- Excellent communication and leadership skills, with the ability to motivate and inspire a team;
- Strong problem-solving and critical thinking skills, with the ability to make sound judgments in high-pressure situations;
- A commitment to delivering exceptional patient care and promoting a positive work environment.
