Key Responsibilities- Respond to and triage calls in a timely manner, documenting all telephone contacts, patient care observations, and interventions.
- Document outcomes of calls, patient/caregiver responses, and any further instructions needed in our electronic medical record system.
- Prioritize calls and complete visits for patients as needed, communicating with and supporting after-hours staff in assessing and treating patient needs.
- Document all interim verbal orders obtained from physicians accurately into the electronic medical record system and communicate with team members to ensure implementation.
- Receive and record accurate and thorough referral information requests for services, notifying the Intake Nursing Staff of referrals.
- Record the outcome of all inquiries and referrals received, providing appropriate follow-up when an inquiry or initial referral cannot be serviced during on-call hours.
- Provide skilled professional nursing care to patients/families/caregivers as prescribed by physicians and in compliance with agency policies when called upon after-hours.
- Complete clinical documentation of all after-hours calls and visits, including problems and interventions within the electronic medical record system timely and in accordance with agency policies and procedures.
- Complete an on-call log on telephone calls as received, submitting on a daily basis.
- Communicate with the interdisciplinary team to ensure proper patient follow-up on previous night's after-hours calls.
- Provide accurate information to customers making telephone inquiries regarding Home Care, Hospice, Personal Home Services, and other agency services.
- Notify after-hours administrators on-call when necessary and/or when guidance is needed.
- Communicate clearly with professional staff regarding expected 'call-backs' and relay information to physicians and others.
- Maintain a positive working relationship with referral sources and individuals seeking information.
- Graduate of an accredited school of nursing, accompanied by a current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC).
- Three to four years of recent general medical/surgical or skilled/acute care nursing experience.
