Skilled Care Nurse

About the Role
As an LPN at Beavercreek Health and Rehab, you will provide direct nursing care to our residents, ensuring their physical and emotional needs are met. You will work closely with our interdisciplinary team to develop and implement individualized care plans that promote optimal health outcomes.
The Ideal Candidate

Responsibilities and Qualifications
Key responsibilities include:
• Providing direct nursing care to residents, including administering medications, monitoring vital signs, and performing treatments
• Conducting thorough assessments of residents' medical and social histories, identifying potential risks and developing strategies to mitigate them
• Collaborating with the healthcare team to create and implement individualized care plans that meet each resident's unique needs
Qualifications include:
• Current licensure as a Licensed Practical Nurse in the state of Ohio
• High school diploma or equivalent required; previous experience in long-term care or skilled nursing facilities preferred
• Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and interdisciplinary teams
Benefits
We offer a competitive salary range of $55,000-$65,000 per year, depending on experience, plus benefits including medical, dental, and vision insurance, paid time off, and a 401(k) plan.
