We're seeking a skilled and compassionate Advanced Practice Clinician to join our HouseCalls team as a Primary Care Clinician. In this role, you'll have the opportunity to make a meaningful impact on patients' lives by providing annual clinical assessments in the comfort of their homes.

As a Primary Care Clinician with Optum, you'll be part of a dynamic team that's revolutionizing healthcare by uniting industry-leading solutions to create an integrated care model that addresses patients' physical, mental, and social needs. You'll work collaboratively with other healthcare professionals to provide comprehensive care that improves health outcomes and enhances patient experience.

Key Responsibilities:

  • Conduct thorough in-home assessments, including medical history review, medication reconciliation, vital signs, and comprehensive physical exams
  • Perform evidence-based practice screenings, including point-of-care testing as appropriate
  • IDentify diagnoses and develop care management plans to address patient needs
  • Communicate assessment findings to patients' primary care physicians to inform gaps in care
  • Respond to urgent and emergent situations, providing timely interventions as needed

You'll also have the opportunity to educate patients on disease processes, medications, and compliance, promoting healthy behaviors and improving health literacy.

In return for your dedication and expertise, we offer a competitive salary range of $88,000 to $173,200 annually, depending on experience, education, and certifications.
