As a key member of our cross-functional care team, you will work closely with patients, caregivers, providers, and other stakeholders to provide integrated health services and achieve optimal health outcomes.

The role of the Healthcare Case Manager involves applying nursing processes to actively manage members, providing education and guidance to help them navigate complex healthcare systems, and maintaining high standards of quality care. This includes adhering to therapeutic standards, measuring health outcomes against member care goals and standards, making necessary care adjustments, and following governing agency regulations.

To be successful in this role, you should have a Bachelor of Science in Nursing (BSN) degree and at least 2 years of clinical experience with direct patient care. You should also be comfortable communicating with members and providers via phone and possess strong administrative/technical skills, including proficiency in Microsoft Office and teleconferencing software.

We offer a competitive salary range of $85,000 - $115,000 per year, depending on experience, as well as a comprehensive benefits package, including health, vision, and dental coverage, life insurance, adoption assistance, and paid time off.
