Care Coordination Director

Nuvance Health is seeking a highly skilled Care Coordination Director to lead our team in providing quality care to patients. As a key member of our organization, you will be responsible for ensuring that patients receive the most cost-effective care possible, while also promoting continuous quality improvement and optimal financial productivity.

The ideal candidate will have a strong background in nursing and management, with a minimum of five years of experience in care coordination and utilization management. A Master's Degree in Nursing or Health Management is desired, but not required.

Responsibilities will include:

  • Providing direct management to care coordination staff, ensuring they have the education and training needed to excel in their roles.
  • Developing and implementing program philosophy and operational goals that align with the organization's mission and strategic plan.
  • Ensuring compliance with Joint Commission, State, and Federal regulations related to discharge planning, utilization management, and continuum of care.
  • Creating and fostering positive customer relationships with patients, physicians, healthcare agencies, and the network.
  • Supporting effective customer service networks in collaboration with the Director of Transition.
  • Identifying opportunities for growth and developing new services and programs.
  • Coordinating services with admission, finance, managed care, medical records, home care, rehabilitative, and risk management.

Requirements include:

  • Bachelor's Level Degree
  • Minimum of five years of job-related experience
  • Registered Nurse with management experience
  • Knowledge of community resources, agencies, financial assistance programs, and/or experience in managed care plans
  • Excellent communication skills
  • Ability to collaborate with multidisciplinary groups and staff at all levels, including community partners
  • Problem-solving skills that achieve specified goals and assist staff to critically think to reach optimum outcomes
  • Fostering teamwork and the ability to integrate the Care Coordination Model and be a change agent
  • Comprehensive knowledge of the healthcare industry, familiarity with government regulations, and possession of strategic planning skills

This is a full-time position with a standard 40-hour workweek, with a salary range of $[insert salary range].
