Responsibilities

As an LPN on our team, your primary responsibilities will include:

• Providing direct nursing care to individuals with intellectual and developmental disabilities
• Conducting health assessments, developing individual care plans, and implementing interventions as needed
• Collaborating with interdisciplinary teams to ensure comprehensive care and support
• Documenting patient progress and outcomes accurately and thoroughly
• Participating in ongoing education and training to stay current with best practices and industry developments

Requirements

To be successful in this role, you will need:

• A valid LPN license in the state of practice
• Minimum of one year of clinically relevant experience in geriatrics or IDD care
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong analytical and problem-solving skills
• Flexibility to adapt to changing priorities and circumstances
