Job Title: Mental Health Service Coordinator

Salary: $60,000 - $80,000 per year

Job Description:

We are seeking a skilled and empathetic Mental Health Service Coordinator to join our team. This role is pivotal in delivering high-quality mental health services to help clients achieve their recovery goals and improve overall well-being.

Key Responsibilities:
  • Conduct comprehensive assessments to identify client strengths, needs, and goals.
  • Develop, implement, and monitor tailored treatment plans in collaboration with clients, their families, and our team members.
  • Provide direct therapeutic interventions, including individual, family, and group therapy, as well as crisis intervention.
  • Coordinate and link clients to necessary community resources.
  • Advocate for clients within various systems to ensure comprehensive support.
  • Regularly monitor client progress and adjust treatment plans to ensure effectiveness.
  • Maintain detailed documentation in accordance with agency and regulatory standards.
  • Collaborate closely with our team members to ensure a holistic approach to client care.
  • Participate in team meetings and case conferences to discuss and coordinate client care.
Additional Responsibilities:
  • Provide psychoeducation as indicated.
  • Assist with crisis interventions and the development of relapse prevention and disease management strategies.
  • Coordinate assessment activities and ensure the implementation of the person-centered plan (PCP).
  • Consult with medical and non-medical providers, engaging community supports and incorporating their input into the planning process.
  • Link clients to clinically appropriate services and arrange for necessary evaluations.
  • Complete functional needs assessments to determine service scope and anticipated outcomes.
Requirements:
  • A Bachelor's degree in a related field.
  • At least 2 years of experience working with individuals with mental health or substance use challenges, preferably in a community-based setting.
  • Strong knowledge of evidence-based practices in mental health and substance use treatment.
  • Excellent communication, organizational, and problem-solving skills.
  • Commitment to person-centered care and cultural competence.
  • Valid driver's license and reliable transportation.
