About the Role:

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team of healthcare professionals. As an LPN at Elara Caring, you will have the opportunity to make a real difference in the lives of our patients and their families.

Job Description:

The Licensed Practical Nurse performs skilled nursing functions in accordance with federal, state, and local laws and within the guidelines of his/her professional organization and agency. The LPN delivers care to clients as delegated by and under the supervision of the Registered Nurse.

Key Responsibilities:

  • Delivering high-quality patient care in a home setting
  • Collaborating with the interdisciplinary team to develop and implement individualized care plans
  • Monitoring and reporting changes in patient condition to the supervising RN
  • Providing education and support to patients and their families

Requirements:

  • Current LPN license in good standing
