We are seeking a dedicated Nurse Coordinator to join our team at Providence Seward Medical Center in Seward, Alaska. This is an exciting opportunity to work in a fast-paced environment and make a meaningful difference in the lives of our patients and their families.
Job DescriptionJob Summary:
The Nurse Coordinator will be responsible for coordinating patient care services, ensuring that all aspects of care are delivered with compassion, respect, and excellence. The ideal candidate will have strong communication skills, attention to detail, and the ability to work effectively in a team environment.
Responsibilities:
- Coordinate patient care services, including admissions, transfers, and discharges.
- Develop and implement individualized care plans for each patient.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and optimize patient outcomes.
- Provide education and support to patients and families on health management and disease prevention.
- Maintain accurate and up-to-date records of patient information and progress.
Requirements:
To be successful in this role, you must possess a graduate degree in nursing and a current RN license in the state of Alaska. Prior experience in a healthcare setting is preferred, but not required.
