In this challenging yet rewarding role as a Clinical Nursing Coordinator II, you will play a vital part in delivering high-quality care to patients with hematological malignancies. Pennsylvania Medicine's Division of Hematology/Oncology is one of the largest groups in the country, recognized nationally and internationally for its excellence in patient care, research, and training.

About the Opportunity

As a Clinical Nursing Coordinator II, you will work closely with a multidisciplinary team to facilitate the coordination of care across the continuum. You will be responsible for evaluating patients' needs, developing individualized care plans, and ensuring timely and effective implementation of treatment protocols. This role requires excellent communication and interpersonal skills, as well as a strong understanding of evidence-based practices and safety protocols.

Key Responsibilities
  • Develop and implement individualized care plans based on patients' needs, preferences, and values.
  • Collaborate with interdisciplinary teams to ensure seamless coordination of care and effective communication with patients and families.
  • Evaluate and improve care processes using data and quality improvement methods.
Requirements

To be considered for this role, you must have a valid RN license, ACLS or BCLS certification, and at least 15 months of relevant professional nursing experience.
