Job Summary

Central Maine Medical Center is seeking a skilled RN Case Manager to join our Home Health Care team. As a RN Case Manager, you will be responsible for assessing, planning, implementing, and evaluating the needs of patients for discharge planning and utilization review.

Key Responsibilities

  • Assess patients' needs for discharge planning and utilization review
  • Develop and implement individualized care plans to meet patients' needs
  • Coordinate with physicians, nursing staff, and other healthcare professionals to ensure seamless care transitions
  • Communicate with patients and their families to ensure they are informed and engaged in their care
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed

Requirements

  • Graduate of an accredited school of nursing with up to six (6) months of Registered Nurse experience
  • Current American Heart Association Healthcare Provider BLS certification
  • Knowledge of criteria for Medicare, Medicaid, HMO, and private insurance coverage
