Comprehensive Care Manager

About the Role
As a Lead Care Manager with Comprehensive Community Health Centers Inc., you will oversee and implement provision of Enhanced Care Management (ECM) services, identifying and achieving SMART goals and objectives with members that meet their self-identified strengths and health care and psychosocial needs.
Key Responsibilities
- Engage eligible ECM members, offering services where they live, seek care, or find most easily accessible within health plan guidelines.
- Connect with ECM members via phone or in-person to facilitate engagement, assessment, follow-up, and education/training visits to develop care plans and address patient needs/goals.
- Work with ECM members to identify SMART goals and objectives.
- Connect ECM members to other social services and supports they may need.
- Advocate on behalf of members with healthcare professionals.
- Use motivational interviewing and trauma-informed care practices.
- Collaborate with hospital staff on discharge plans.
- Accompany ECM members to office visits as needed and according to health plan guidelines.
- Monitor treatment adherence, including medication.
- Provide health promotion and self-management training.
- Arrange transportation.
- Convene care conference meetings.
- Coordinate with ECM resource partners to obtain data/information for accurate care plan updates.
- Utilize electronic tracking systems to coordinate services and input data for reporting.
- Supervise Community Health Workers.
- Attend required ECM trainings.
- Facilitate internal ECM trainings/updates for CCHC clinical and administrative staff members.
- Collaborate with the IT department to ensure ECM templates are effective at capturing needed data/information.
- Review existing protocols and policies to match ECM requirements.
- Develop new ECM protocols, policies, and accompanying workflows.
- Assist the ECM Director in submitting required ECM reports and internal reports.
- Other duties as assigned.
Requirements
The ideal candidate will have 3-5 years of experience providing home health/case management services to low-income populations with complex chronic conditions, high utilizers of emergency room and tertiary healthcare services, severe mental illness, and/or homelessness.
Bilingual skills in English/Spanish are preferred.
Estimated Salary Range: $44,800 - $48,000 per year
