Salary and BenefitsWe offer a competitive salary range of $ 90,000 to $110,000 per year, depending on experience, along with a comprehensive benefits package that includes health, dental, and vision insurance, 401(k) matching, and paid time off.
Job DescriptionSummary: As a Physical Therapist at JAG Physical Therapy, you will be responsible for assessing patients' conditions and developing customized treatment plans to promote healing and improve functional independence.
Duties and Responsibilities:
- Collects and records patient care data, including initial evaluations, daily notes, reassessments, progress reports, and discharge plans.
- Develops and implements treatment plans based on evaluation results, goals, and best practices, collaborating with interdisciplinary teams and patients.
- Provides education and training to patients and their support systems on ADLs, therapeutic activities, rehabilitation equipment prescription, and community reintegration.
- Administers tests and measures, synthesizes evaluation results, and formulates diagnoses, goals, prognosis, and plans of care.
- Initiates and completes discharge plans, referring patients to other healthcare professionals or services as needed.
To be successful in this role, you will need:
- A Bachelor's or Master's degree in Physical Therapy, preferably a Doctorate degree.
- A valid Physical Therapist license in the state where you will practice.
- Completed state-regulated CEU requirements in New York or New Jersey.
- Valid CPR certification.
- Experience with EMR software is preferred, but not required.
- Excellent communication, clinical decision-making, and teamwork skills.
