Key Responsibilities:

  • Assess, implement, and evaluate a treatment program for home care patients to develop communication, feeding, swallowing, developmental, and cognitive abilities.
  • Coordinate care provision with the multidisciplinary team to assure agreed upon outcomes for care.
  • Provide evaluations and ongoing assessments as a member of a multi-disciplinary team and provide trainings for staff and/or families.
  • Provide speech therapy services to Pediatrics who have language delays, feeding delays, cognitive impairments, trauma, injuries or other conditions requiring Speech Therapy.
  • Demonstrate knowledge of normal and abnormal communication development; speech therapy intervention techniques; identification, evaluation and diagnosis of communication disorders.
  • Use and care of assistive devices used in speech and language therapy.
  • Responsible for completion of timely documentation including progress notes, reports, care plans, evaluations, and Medicaid authorizations.
  • Communicate effectively with families, members of the Individualized Family Service Plan (IFSP) team, and other health care providers regarding diagnosis and therapy.
  • Maintain punctual and dependable scheduled visits.
  • Participate in the development of the IFSP and makes recommendations/treatment plans for the overall care plan for the family.
  • Appraise parents' participation and instruct and train parents in programs individualized to their child.
  • Adapt to different and changing environments and settings.

Requirements:

  • Must be licensed in the State of Colorado as a Speech Pathologist.
  • Must hold a master's degree in Speech/Pathology.
  • Must hold Certificate of Clinical Competence in Speech/Language Pathology from the American Speech/Language and Hearing Association or equivalent.
  • Proof of professional liability insurance if a contracted service; proof of health/immunization if a contracted service.
  • BLS certification required.
  • Certification for Colorado Early Intervention services.
