Chronic Disease Management Specialist

We are seeking a skilled RN Case Manager to join our team in the South East Atlanta, GA region. This is a full-time position that requires strong communication and organizational skills.

Responsibilities- Coordinate care for patients with complex chronic illnesses, including CKD/Eskd, through medical record review and consultation.
- Identify medical, social, emotional, and financial needs of patients and implement appropriate interventions.
- Work autonomously and collaboratively with healthcare teams to facilitate quality, consistent, and cost-effective care.
- Current RN license required; BSN preferred.
- Minimum 2 years RN experience, with healthcare performance coaching.
- Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification.
- Ability to travel within an assigned territory.
- Intermediate computer skills and proficiency in MS Word, Excel, and Outlook.
- A competitive salary range of $73,000 - 85,000 per year.
