Key Responsibilities- Provide pharmacokinetic consultation and medication therapy management to patients and healthcare providers.
- Evaluate patient response to drug therapy and develop strategies to optimize medication regimens.
- Collaborate with interdisciplinary teams to develop and implement population health initiatives.
- Develop and maintain educational programs for healthcare providers and patients on medication management and population health topics.
- Stay up-to-date on current federal, state, and local laws, accreditation standards, and regulatory agency requirements that apply to the assigned area of responsibility.
- Current Pharmacy license in the state of practice.
- Completion of a PGY1 Pharmacy Practice Residency (3 years related specialty experience accepted in lieu of PGY1 Residency).
- Preferred: Doctor of Pharmacy (PharmD) and completion of a PGY2 Pharmacy Residency.
This position requires the ability to work in a fast-paced environment and adapt to changing priorities. The incumbent will work with patients who have known or suspected communicable diseases and may enter isolation rooms. The incumbent has an occupational risk for exposure to all communicable diseases.
