Job Summary

Pace is seeking a highly skilled and compassionate professional to fill the role of Intake and Transition Services Specialist. This position is responsible for providing critical support to girls and their families as they navigate the Pace Program.

Key Responsibilities
  • Serve as the primary point of contact for families and girls considering the Pace program, providing guidance and support throughout the intake process.
  • Conduct thorough needs assessments and collaborate with the intake team to determine the suitability of placement for each girl.
  • Compile and maintain accurate documentation in accordance with agency policies and procedures.
  • Participate in intake interviews to assess the needs of each girl and family, working closely with the intake team to develop personalized plans.
  • Assist in maintaining center census and ensuring a smooth transition process for new girls entering the program.
  • Represent Pace to referral sources, promoting the program's mission and services.
  • Facilitate orientation and provide ongoing support to girls as they transition through the program.
  • Collaborate with the care review team to plan for ongoing transitional care management and ensure a seamless transition to aftercare services.
  • Coordinate aftercare services, monitoring progress and providing support to girls as they transition into life roles.
  • Provide career counseling and assistance to girls, helping them to secure employment and achieve their goals.
  • Participate in family conferences and establish positive relationships with outside agencies to ensure girls receive the support they need.
  • Monitor and record progress, documenting all areas of assessment in accordance with Pace Center Standards and regulatory agencies.
Qualifications and Requirements
  • Bachelor's Degree in Social Work, Psychology, Mental Health, Counseling, or a related field.
  • Minimum three years of prior experience in case management and counseling.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Proficiency in using personal computers and basic office equipment.
  • Current Florida driver's license.
Preferred Qualifications
  • Knowledge of laws relating to children and families.
  • Experience working with at-risk youth.
