Medical Social Work Specialist

About the Role:
We are seeking a skilled Medical Social Work Specialist to join our team at Royalty Staffing. As a key member of our healthcare integration team, you will provide essential social work services to patients and their families, collaborating with healthcare providers to ensure seamless care transitions.
Responsibilities:
- Assessment and Care Planning: Conduct thorough assessments of patients' psychosocial needs, develop and implement personalized care plans, and coordinate services to address these needs.
- Case Management: Manage complex cases, providing advocacy services, resource identification, and referral support to patients and their families.
- Program Development and Evaluation: Assist in the development, implementation, and evaluation of social work programs and policies, ensuring alignment with departmental goals and objectives.
- Consultation and Education: Provide consultation and education to healthcare staff on social work best practices, community resources, and emerging trends in medical social work.
- Research and Quality Improvement: Participate in research activities, design studies, collect and analyze data, and implement quality improvement processes to enhance patient outcomes and departmental performance.
- Collaboration and Communication: Foster strong relationships with community agencies, healthcare providers, and other stakeholders to ensure effective coordination of care and service delivery.
Requirements:
- A minimum of one (1) year of experience in medical social work, preferably in a healthcare setting.
- Licensed Social Worker (Hawaii) or Licensed Clinical Social Worker (Hawaii) certification.
- BLS certification from the American Heart Association required.
Preferred Qualifications:
- Prior experience working in medical settings, particularly with patients having various medical diagnoses.
- Demonstrated ability to network effectively with community agencies and healthcare providers.
Benefits and Compensation:
This role offers an estimated annual salary of $65,000-$85,000 based on location and experience. In addition to competitive compensation, we provide a comprehensive benefits package, including medical, dental, and vision insurance, retirement plan, and paid time off.
